MA requires all employers to report new hires and newly hired independent contractors to the state within 14 days of their first day of work. If you have fewer than 25 employees, use this form to report new hires and new independent contractors and submit via mail or fax to the MA Department of Revenue. If you have 25 or more employees you must report your new hires and new independent contractors online through MassTaxConnect.
